You can schedule an appointment by calling our main line at (800) MED-CARE, using our online appointment form, or through the patient portal. For new patients, we recommend calling directly so we can gather your information and match you with the right provider.

We accept most major insurance plans including Blue Cross Blue Shield, Aetna, Cigna, UnitedHealthcare, and Medicare. Please contact our office to verify your specific insurance coverage before your appointment.

Patients can access their medical records through our secure patient portal. You can also request physical copies by completing a medical records release form and submitting it to our office. Please allow 7-10 business days for processing.

For your first appointment, please bring your photo ID, insurance card, a list of current medications, your medical history, and any relevant medical records or test results. Please arrive 15 minutes early to complete necessary paperwork.

Yes, we offer telehealth appointments for many types of visits. These virtual consultations allow you to meet with your provider from the comfort of your home. Not all conditions can be treated virtually, so please call our office to determine if telehealth is appropriate for your needs.

We request that you notify us at least 24 hours in advance if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient who may need urgent care. Repeated no-shows or late cancellations may result in a cancellation fee.
